Off-Campus Housing ----- Incident and Complaint Form

Each housing report you provide is important and valuable to our department and the university. Our goal is to help managers and owners gain valuable insights, from the information you provide, so they can seek to improve their properties and create the best learning environments possible for our students.  If you have been involved in a housing incident, have an important concern or complaint , or just want to provide feedback with off-campus housing, please fill out the form listed below.

All concerns are taken very seriously. This form allows students and community members to report incidents, concerns and complaints in a safe environment. When filing a "Housing Complaint / Feedback Form," your concern will be received and recorded with our department. Certain information is then forwarded on to various managers, landlords, and local government authorities with the goal that each complaint and incident will be heard and properly addressed, and that all local housing laws will be sustained.

Outline the details of your complaint and/or feedback in a "TIMELINE" fashion. Please answer the following where possible: 1 - Dates and times of incident(s) 2 - Location of incident(s) - Provide accurrate Address, Apartment Number and Apartment Name. 3 - Manager / Landlord's Name 4 - Management / Landlord's Office Phone 5 - Management / Landlord's Email Address 6 - Identify all parties involved (FULL NAMES if possible) 7 - (Part 1) Were local police involved? 7 - (Part 2) Was there an official police report? 8 - Has the incident occurred before? 9 - What have you done to resolve the concerns? 10 - Has the management / landlord been previously notified about this issue? 11 - Please describe how the management / landlord attempted to solve the problem. 12 - Please describe if and how the housing contract has been a breached?

I hold Utah Valley University, its employees and students, as well as local off-campus managers and landlords, local police, and local governments and their designees blameless if for any reason this information should be fully disclosed or used improperly. I release the university, including all employees and students, from all liability for my issuing this private and/or public information relative to this form. I authorize Utah Valley University and the Department of Housing and Residence Life to make any changes and/or alterations to any and/or all parts of this form at any point before or after completing and submitting this form. I will hold Utah Valley University and/or any of its designees blameless and not responsible for any partial or full damages made by any and/or all changes or alterations made to this form. I understand and certify that all information entered into this form is voluntarily provided and is true to the best of my knowledge. I fully understand and agree that I am submitting any and all information within this form willingly, of my own accord, and I fully understand and agree that any or all private and/or public information shared in this form may or may not be used without any restrictions. I fully understand and accept that Utah Valley University does not own or manage any housing, on or off-campus, and I fully understand and accept that Utah Valley University is limited in what jurisdictions and services it can, will, and may provide. I agree that this form does not constitute any legally binding contract and that Utah Valley University does not legally represent any and all person(s) and/or organization(s) in their housing through this form. In completing this form, I fully understand and accept that Utah Valley University does not make any promises or contracts that the university, off-campus housing managers and landlords, and/or local authorities will handle emergencies, problems, concerns, complaints, incidents, and/or issues immediately or correctly. I fully understand and accept that this form does not release me from my personal duties and responsibilities of involvement and that this form may or may not assist in my efforts to reach out to certain organizations, businesses, governments, and/or persons. I fully understand and accept that it is my personal duty and responsibility to lawfully handle any and all emergencies, concerns, complaints, issues, and problems within off-campus housing and that I must take full responsibility to contact appropriate authorized authorities, including off-campus housing managers and landlords, local police, and local city authorities to handle any and all emergencies, concerns, complaints, issues, and problems within off-campus housing, regardless of the information received in this form and regardless of this form being provided by Utah Valley University.
